A portmanteau of fentanyl and analog, referring to synthetic opioids chemically similar to fentanyl but slightly altered in molecular structure — often to sidestep drug scheduling laws. Fentalogs are at the center of the overdose crisis because they can be dozens to thousands of times more potent than morphine, and users often can't tell them apart from other substances. The term is used in harm reduction, law enforcement, and drug policy discussions.
The toxicology report came back positive for a fentalog — not fentanyl itself, but close enough to be just as deadly.
